Gather up the documentation and information described and listed below so you can complete the form properly (some of the info/documentation will be in the package of docs you received when you originally bought your home).

You should have a copy of your most recent property tax bill which will have your parcel or property id # and legal description. Measure the main rooms in your home, count the number of ceiling fans, make a list of the equipment and appliances. If you don't know for sure, get together the info on your homeowner maintenance fees (amount per month, quarter, etc), any restrictions on pets, age, renting of your property, application fees, etc. Your association or association docs should have this info. Also be sure to have your association contact info available.
